This is an exciting time to be part of our developing and progressive working environment which is dynamic and proactive with a commitment to making a difference for our patients and carers.
The extended primary care team consist of Physiotherapist, Occupational Therapists, Care Navigator and Admin. The Nurses component has the emphasis on experience senior nurses working with families to promote independent living.
The Therapy team includes 1 Band 7 PT or OT, Band 6 OT and Physio, and 3 Rehab support worker in each team. It works closely with the integrated team of nurse’s team who has 1 Band 8a, 2 Band 7, 5 Band 6 nurses making up the senior nurse team. They are supported by 6 and 5 nurses and 5 HCA with advance nursing skills as well Band 7 community mental health nurse. This gives good peer support and development to the team. The integrated team across social services, voluntary sector and health input make up the team to plan the patient focused care plan.
As well as bringing your clinical expertise, you will be a champion of integrated community care. You will be a strong team player, an excellent communicator who is adaptable, committed, well organised and work well under pressure.
You will support the development, implementation and maintenance of high quality, safe and sustainable clinical services and provide clinical and operational leadership under the direction of the team leaders, clinical lead and general manager.
