An exciting opportunity has arisen for a nurse with community experience to join the Tower Hamlets Intermediate Care In-reach team. Working primarily with ward teams at the Royal London Hospital, Whitechapel, the In-reach nurses provide a link between hospital and community, assisting ward teams with complex discharges.
The role also involves case finding patients in hospital who are appropriate for intervention from the Community Intermediate Care and assessing patients who require IV antibiotic treatment in the community.
Candidates should have a wide range of clinical skills, including IV administration, venepuncture and cannulation and knowledge of tissue viability and pressure ulcer prevention and treatment. Experience of the discharge to assess model is an advantage.
You must have appropriate UK professional registration.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
